Managing Continuity

The theme of managing continuity across the health system reflects concerns about effective co-ordination and integration of care and service across organizations, sectors and regions (e.g., across institutional care, homecare, long-term care and community care, and between regionalized services and local primary healthcare. Research under this theme may evaluate strategies for managing system continuity across two or more points of the health system/service continuum. (e.g., gatekeeper roles, shared health professional coverage, patient rostering, placement or discharge coordination services). Managers and policy makers consistently identify strong interest in evaluating organizational and financial strategies for managing continuity.
